This cookie is set to remember your patient code so that if you submit more scores we can link it to past scores.

If you do not wish to be tracked in this way then do not submit data.

This cookie is vital in the functionality of this service to keep you anonymous.

What data is collected and why is it stored?

How is the information processed?

The information gathered will be used by the surgeon. They will inform you how they plan to use the data.

This system does not sell your data. It makes the data avaliable to the surgeon that the surgeon code refers to and nobody else. Contact them for information on how they handle your data.

Deletion and retention

Submitted data is stored temporarily on cloud based servers and is downloaded to a secure database on a weekly basis. Following download, the data is deleted from the cloud based servers..

Data security

The data is encrypted at rest. The data can only be downloaded with knowledge of a secure signing key.
